Direct Primary Care (DPC) is an innovative alternative payment model improving access to high functioning healthcare with a simple, flat, affordable membership fee.
Individual 18 year and older $75 per month
Family of 2 $100 per month
Family of 3 $ 130 per month
Family to 4 people $150 per month
Each additional family member $25 per month
All plans include unlimited office visits and evening hours.
This month I am waiving the $99 enrollment fee.

What Does Your Membership Cover?
Primary Care Membership Monthly Fee Includes:
1. Primary care visits including but not limited to annual exams, problem visits, and work/school physicals.
2. Acute illness/ Injury needs including but not limited to exam/diagnosis, lacerations, wound care, respiratory care, fractures/ sprains, and so much more.
3. Sick calls via tele-medicine or phone call.
